Last month, information regarding the vaccine requirement reporting was announced in 抖阴旅行射 Boulder Today. As a supervisor or HR liaison, we ask for your assistance in reminding faculty and staff that action in the My抖阴旅行射Health Portal is required by Sept. 15, 2021.听

All faculty and staff need to submit their COVID-19 vaccine information or their exemption form to complete the 抖阴旅行射 system COVID-19 vaccine requirement. This process must be completed by logging into the My抖阴旅行射Health Portal. Detailed instructions are available through Medical Services.

1. Who needs to take action?聽

   1. The university previously communicated that faculty and staff vaccinated in Colorado would not need to report their information to 抖阴旅行射, and that has changed. All faculty and staff must complete the COVID-19 vaccine requirement or exemption in the My抖阴旅行射Health Portal. The only exception will be faculty and staff vaccinated through 抖阴旅行射 Boulder Medical Services. Data from Medical Services will be uploaded to the portal on behalf of faculty and staff vaccinated through Medical Services.

2. Can I ask students in my class or co-workers if they鈥檝e been vaccinated?聽

   1. No. Students, staff and faculty should not inquire about an individual鈥檚 vaccine status, which is considered protected health information.

3. What are medical and non-medical exemptions?聽

   1. Faculty, staff and students who cannot or who choose not to get a vaccine can complete an exemption.

   2. Note: As with all required vaccines, in the event of an outbreak, exempted persons may be subject to exclusion from 抖阴旅行射 Boulder and quarantine. The specifics may vary depending on the circumstances of the outbreak.

      1. Medical exemption: Medical exemptions must be signed by a licensed physician and must include a date and a reason for exemption.

      2. Non-medical exemption: Non-medical exemptions are provided for religious or personal beliefs that are opposed to immunizations.

4. If I work remotely, do I need to complete the vaccine requirement?聽

   1. This requirement is for all students, staff and faculty.听

5. I am a new employee, when can I get into the My抖阴旅行射Health portal to take action?聽

   1. 抖阴旅行射 adds new employees to the vaccine verification system within 1-3 days from your start date.听 If you log into the system and receive a blank screen, you have not been uploaded yet.
